A new look at varicose veins surgery - CHIVA, an alternative treatment

Professor Mark Whiteley has just returned from a 4 day course about CHIVA surgery for varicose veins in Cremona, Italy with Prof Claude Franceschi, Dr Roberto Delfrate and Dr Massimo Bricchi.

CHIVA is a technique of treating varicose veins developed by Prof Claude Franceschi which essentially diverts blood rather than destroying veins.

Dr Roberto Delfrate marking a patient with Duplex before CHIVA
There are several potential advantages of this system - but the main advantage is the saving of the main saphenous vein in the leg, which is then available for bypass surgery later in life if needed.

Leg Ulcer Charity website launched

The Leg Ulcer Charity is a new UK charity whose mission is to reduce the number of patients in the UK with leg ulcers.

http://www.legulcercharity.org/
Leg Ulcer Charity website launched - www.LegUlcerCharity.org

Research over the last decade has shown that most leg ulcers can be cured by proper investigations and local anaesthetic endovenous surgery.

Unfortunately, most patients only get offered bandages and dressings which might temporarily improve or heal the leg ulcer .... but as they do not affect the underlying causes, do not cure the problem in the long term. Hence the ulcer returns.

Using proper modern medical investigations, specialists can find the underlying cause of the leg ulcer in the majority of patients and permanently fix it - curing the leg ulcer for good.

The Leg Ulcer Charity has been set up to:

  1. EMPOWER patients, family and carers to ask for the right investigations and treatments to get a cure of a leg ulcer
  2. RESEARCH new leg ulcer investigations, treatments and effects that having a leg ulcer has on patients, family and society
  3. EDUCATE doctors and nurses to now about new methods to cure leg ulcers permanently
  4. Provide GRANTS to patients needing help to get their leg ulcer investigated and treated

Any donations or sponsorship can be made through the website.

Ultrasound best to assess pelvic varicose veins - Whiteley Clinic research presented at the VSS

Scott dos Santos, the Whiteley Clinic research co-ordinator, presented an original research study last evening at the national meeting of the Vascular Surgical Society of Great Britain and Ireland.

Scott dos Santos, Whiteley Clinic research co-ordinator presenting
a paper at the Vascular Surgical Society, Nov 2013

Scott's research study showed that when assessing pelvic varicose veins in women, the trans-vaginal ultrasound scan is the Gold Standard test.

Many doctors still use MRI, CT or venograms - however this research suggests that these other tests will leave some patients with incorrect diagnosis and that might lead to the wrong or incomplete treatment.

This research has been submitted for publication so that other doctors and clinics can use the findings of The Whiteley Clinic's research efforts.

False aneurysm in groin after EVLA treatment for varicose veins - a rare complication

Any complication after EVLA laser treatment for varicose veins is rare when done in a specialist vein clinic.

However we have just published a fascinating case of an extremely rare complication.

Arterial false aneurysm in the groin following endovenous laser ablation
Ostler AE, Holdstock JM, Harrison CC, Whiteley MS.
Following EVLA treatment, our patient developed a tense "pseudoanerysm" in the groin - bleeding from a local artery.

At The Whiteley Clinic we have an emergency telephone system and so the patient was able to come straight back again. Our specialist vascular technologists were able to identify the precise cause of the problem and the patient was cured by injection of a drug to help clotting and precise pressure on the bleeding vessel by the duplex ultrasound probe.

Perforators and pelvic veins causing varicose veins - Mark Whiteley speaking at Veith, New York

Varicose veins affect are seen in about 20 - 25% of the adult population and so it is essential that when they are treated, the very best techniques are used, in order that the risks of them recurring after surgery is minimised.

Prof Mark Whiteley speaking at the Veith Symposium, in New York, Nov 2013
Photo - Courtesy: Vascular News

Research from The Whiteley Clinic has made it clear that just treating the varicose veins in the legs is not enough for optimal treatment in many patients.

Our research has shown that leg varicose veins are not only associated with the usual long veins that run up the leg, but also come from Incompetent Perforating Veins and Pelvic Veins in many patients. This suggests that patients who only have the leg veins investigated and treated are much more likely to get their varicose veins back again after treatment - varicose vein recurrence.

Most doctors in the world still only provide simple investigation and treatment of the main leg veins, and do not look for, or treat, incompetent perforating veins nor pelvic veins. At The Whitely Clinic we use The Whiteley Protocol - making sure that everyone gets a full investigation and all of their vein problems are treated. This is why our patient satisfaction is so high and our audited results show such a low rate of recurrence after our treatments.

Prof Mark Whiteley was asked to present his research and views at the Veith Symposium in New York in the form of 2 debates and one lecture:

Phlebitis - scan needed to select correct treatment

Phlebitis is a very mis-used word - and leads to incorrect treatments. Antibiotics are NOT the right treatment for phlebitis.

Phlebitis can cause a Deep Vein Thrombosis (DVT) or potentially fatal pulmonary embolism (PE) and so now new guidelines from the UK and USA are out to make sure people get the right treatment.

Before the correct treatment can be selected, it is essential to have a duplex ultrasound scan:
  1.  To confirm the diagnosis of phlebitis
  2. To see the extent of the clot
  3. To choose the optimal treatment

Job opportunity - Entrepreneur wanted to grow online company

There is a great opportunity for a motivated person to head up our on-line College of Phlebology.

No knowledge of veins is needed - we have all the experts on tap that you will need!

Entrepreneur needed to grow, and be part of, The College of Phlebology

Over the last decade, vein surgery has changed from being in hospitals under general anaesthetic to being a local anaesthetic walk-in, walk-out procedure. Using new techniques such as endovenous laser ablation (EVLA), radiofrequency ablation (RFA), foam sclerotherapy, glue and Clarivein (MOCA), there has been an explosion of vein clinics and vein doctors around the world.

Centred in the world famous Whiteley Clinic in Guildford, the College of Phlebology is an internet portal about veins with a public facing and professional facing role.

We are looking for a motivated person with huge enthusiasm to co-ordinate with Prof Mark Whiteley, his team, the researchers and other associated vein experts around the world as well as with companies involved in venous equipment, to grow this exciting project.

It is envisaged that the successful candidate will start as business manager and, as the company rows under their care, to take the CEO role.

This is a great opportunity for someone at the start of their business career to hit the ground running and grow a company which has already been established. It might also suit a more experienced person who has had previous success and now wants a new challenge.

We do not want advisors please - only someone dedicated to make it happen themselves!

DVT missed - All leg vein problems need duplex ultrasound scans

In the news today was another case of a doctor missing a DVT with tragic consequences.

Veins specialists at The Whiteley Clinic have spent years explaining that no matter what doctors or nurses say, when I comes to vein problems in the legs, if they don't have a duplex ultrasound result in front of them, they are at best making an "informed guess".

Duplex ultrasound is non-invasive and so does not hurt at all and uses no damaging radiation - it is only a specialised ultrasound with no risk at all to the patient.

Whether the problems be:
  • Deep vein thrombosis
  • Phlebitis
  • Varicose veins
  • Leg ulcers
  • Venous eczema
or a host of other leg vein problems, it is highly unlikely that any doctor or nurse will guess the correct treatment without getting a duplex ultrasound scan first to find out what is going on and how extensive the problem is.

If patients with these problems are not offered a duplex ultrasound scan by their doctor or nurse, they should insist on being referred to have one.

Varicose veins - treatment of perforators using TRLOP

Incompetent perforating veins (IPV) are not commonly treated by most doctors who "do" varicose veins. As a result, incompetent perforating veins are found to be one of the commonest causes of why veins come back again after treatment.

In 2001, Prof Mark Whiteley and Judy Holdstock of The Whiteley Clinic invented TRLOP - TRansLuminal Occlusion of Perforating veins:

The TRLOP technique has been shown to be as effective as the old general anaesthetic SEPS (sub-facial endoscopic perforating vein surgery) procedure - but TRLOP is performed:
  • under local anaesthetic
  • using a pin-hole only (no stitches)
  • as a walk-in walk-out or "ambulatory" procedure
This video showing Prof Mark Whiteley and Judy Holdstock performing a TRLOP procedure was shown at the UIP meeting in Boston in September 2013.

Daily Mail on Varicose Veins - Support Stockings are a Waste of Time

An article on varicose veins and the new NICE (National Institute for Health and Clinical Excellence) guidelines appeared in the Daily Mail, quoting Prof Mark Whiteley.

Prof Mark Whiteley quoted in Daily Mail

The article explains how support stockings are not a cure for varicose veins and should not be recommended instead of fixing the varicose veins properly - with Endovenous Laser, Radiofrequency or if unsuitable, with Foam Sclerotherapy.

This problem was previously highlighted by Prof Mark Whiteley in Dec 2012 in an article in The Huffington Post called:"Support Stockings - A 'Sticking Plaster' for the NHS?" (http://www.huffingtonpost.co.uk/mark-s-whiteley/support-stockings-a-stick_b_2330053.html).

Mind-Blowing Discovery about Varicose Veins - Huffington Post

Varicose veins are caused when the one-way valves inside the veins fail, allowing blood to flow the wrong way down the vein, stretching the vein wall. If left untreated, this abnormal flow can cause other problems in a proportion of people, resulting in inflammation, skin damage, eczema, ulcers or blood clots.

Read Prof Mark Whiteley in The Huffington Post

Many doctors have tried to repair the failed valves - with very little success. Others try to strengthen the vein wall, to bring the valve leaflets closer together to try to help the valve to work again - once again with very poor results.

The failure of repairing the valves themselves or the vein holding the valves, suggested there is another reason that the valves fail.

Tortuous veins - How to pass an endovenous catheter to treat Varicose Veins - College of Phlebology

Many doctors who are new to endovenous surgery ask how best to pass an endovenous laser (EVLT) fibre or a radiofrequency ablation (RFA) catheter up a tortuous truncal vein - usually the Great Saphenous Vein (GSV).

In this College of Phlebology video, Professor Mark Whieley describes different methods to get around this problem.

A tortuous vein should never be the cause of a failed endovenous operation and should never stop doctors from treating varicose veins with endovenous laser or radiofrequency.
