Phlebitis and haemosiderin from untreated varicose veins

This 46 year old man came to The Whiteley Clinic having had varicose veins for years. He had not had them treated as "they did not cause him any trouble".

Three weeks ago he suddenly had discomfort and inflammation over his right shin - with hard tender lumps where the varicose veins had been:

 
Superficial thrombophlebitis and haemosiderin complicating varicose veins
that should have been treated before these problems occurred 
Once the lumps and tenderness had appeared, he realised that he needed his veins treated and so chose The Whiteley Clinic becuase of our reputation for excellence.

On examination however, we noticed that he also had brown stains around his ankles. This brown discoluration is called "haemosiderin" deposition and is due to the skin being damaged from long term varicose veins (or "venous reflux") that haven't been treated.

The patient underwent a specialist venous duplex ultrasound scan by a Whiteley Clinic trained expert vascular technologist.

This showed they underlying varicose veins that had caused both of these problems - and would go on to cause further damage and leg ulceration if left untreated.

As with most people suffering from such conditions, the venous duplex ultrasound showed that this patient can be cured using the local anaesthetic walk-in walk-out surgery.

By following The Whiteley Protocol, a combination of EVLA (Endovenous Laser Ablation), TRLOP (TRansLuminal Occlusion of Perforators), phlebectomies and foam sclerotherapy will cure this patient, reversing most of the damage and preventing the further deterioration of the leg towards more phlebitis, skin damage and ulcers.

Leg Ulcer cured at The Whiteley Clinic

Leg ulcers can often be cured using local anaesthetic surgery, following The Whiteley Protocol (TM).

 
 
These photos show a patient with a left leg ulcer (above) who underwent local anaesthetic vein surgery by Mr Barrie Price at The Whiteley Clinic in October 2012.
 
Using EVLA (Endolvenous laser ablation) and TRLOP (TRansLuminal Occlusion of Perforators) the patient had complete cure today - just under 3 months later.
 

Unlike compression treatment with banadages or stockings, the ulcer will stay healed and did not need any bandages or compression during the healing process.

CocaCola and Garlic for varicose veins! - Rubbish on the Internet


The internet has been liberating for many - Indeed it has helped The Whiteley Clinic get the results of their research and new techniques to treat varicose veins into the public arena for the good of patients.
 
Therefore it is always upsetting when rubbish appears - and particularly when the author of the nonsense doesn't allow any comments on the video! 
 

 
This video, made by in Spanish, extols the virtues of CocaCola and Garlic to get rid of varicose veins.
 
When first viewed, Mark Whiteley and the venous specialists at The Whiteley Clinic thought it was a joke and were waiting for the punchline ......
 
Unfortunately it does appear to be serious.
 
Hopefully most people viewing this video will see it for the rubbish it is and hopefully no-one will come to harm by thinking it has any scientific basis.

Unreliable Marriott – book at your risk

It should have been a great birthday - a suite with a large room in the stadium hotel at The Marriott in Twickenham - to celebrate a 50th Birthday AND see England beat The All Blacks. The room was booked with the Marriott online service, with a no refund policy, supposedly giving both parties a guarantee of contract.

So imagine the disappointment on Friday evening, after a very long week at work, to arrive at the Marriott and to be given the wrong room! Surely a mistake??

A phone call to the "supervisor" on call at the hotel yielded "Oh Yes, I can see your booking. I don't know how that happened but the room you booked now has someone in it so there is nothing I can do!"

A frank discussion resulted in the supervisor saying he would reduce the price of the room - but as I pointed out, it would have been about that price if I had booked the smaller room in any case - hence there was no "gift" at all!

He also promised drinks to be sent up for free as an apology - no drinks arrived all weekend.

You may think this is whinging at that "sometimes things happen". But this is the second time this year. So why did I give Marriott a second chance?

In March, I arranged to rake my daughter to watch England as a special treat. I booked the room months ahead, paid for it up front with a no-refund policy, guaranteeing the room.
Forty-eight hours before-hand I was informed at 6pm at night (presumably to prevent a call back) that I was being downgraded to a double room!

After several terse communications, during which I pointed out that I was not going to be sharing a double room with my 19 year old daughter and after having to complain to the RFU itself, the room was re-instated with very bad grace. Four beers and a plate of old sandwiches was sent on that occasion as a "sorry" - no refund of course - but a lot of very nice words reassuring me that the same would never happen again.

Unfortunately I trusted the Marriott - and was let down this second time - the birthday weekend being ruined.

Of course I do not intend to stay at the Marriott again - my rugby at Twickenham will continue but I will not be putting money into the hands of a company that cannot keep to a contract and cannot look after its guests.

I am sure that being associated with the RFU and stadium, they will never be short of people willing to pay for rooms for the rugby and so my own small protest and withdrawal of custom will not make a difference.

However, if you are one of those booking your event with Marriott and, like me, you get let down with little or no warning, then you will not be the first.

New Member on Council of The Venous Forum

Mark Whiteley was delighted today to be invited to sit on the Council of the Venous Forum.

The Venous Forum is a section of the Royal Society of Medicine (RSM) and is the UK’s national venous professional group.

With a Spring Meeting each year in April and a combined meeting at the Vascular Society in November, the Venous Forum currently presents venous surgery in the UK.

Mark Whiteley looks forwards to his time on the Council and hopes to use the position to increase interest and understanding in Venous Disease and Venous Treatments, as well as to help promote the good work of the Venous Forum.

Hyperbaric Oxygen diving tank for Leg Ulcers - but only some

A report in the Daily Mail today shows the benefit of hyperbaric oxygen - or the "Diving Tank" - for non-healing leg wounds - or "leg ulcers" as they call them ( http://www.dailymail.co.uk/health/article-2238941/The-diving-tank-heal-leg-ulcers--help-migraines-too.html ).

Hyperbaric oxygen from a "Diving Tank" is very useful for such chronic wounds - but unfortunately the article doesn't make clear that they are talking about a very small number of leg ulcers.

Of the 500,000 people in the UK who keep getting leg ulcers, the vast majority (some 80-90%) are due to Venous Problems - Venous Leg Ulcers.

In these people, the "Diving Tank" has no benefit. The best way to treat these is to perform local anaesthetic surgery to the veins - not only healing the leg ulcers but stopping them from comming back again.

In our latest published research on leg ulcers, we cured 84% of leg ulcers by local anaesthetic vein surgery.

Abstract:
OBJECTIVES:
This is a retrospective study over 12 years reporting the healing rates of leg ulcers at a specialist vein unit. All patients presented with active chronic venous leg ulcers (clinical, aetiological, anatomical and pathological elements [CEAP]: C6) and had previously been advised elsewhere that their ulcers were amenable to conservative measures only.
METHOD:
Seventy-two patients (84 limbs) were treated between March 1999 and June 2011. Patients were contacted in August 2011 by questionnaire and telephone. Of 72 patients, two were deceased and two had moved location at follow-up, so were not contactable. Fifty patients replied and 18 did not (response rate 74%), representing a mean follow-up time of 3.1 years.
RESULTS:
Ulcer healing occurred in 85% (44 of 52 limbs) of which 52% (27) limbs were no longer confined to compression. Clinical improvement was achieved in 98% of limbs.
CONCLUSIONS:
This study shows that a significant proportion of ulcers currently managed conservatively can be healed by surgical intervention.

Dr Martin Scurr forgot surgery can cure Leg Ulcers

In an article yesterday in The Daily Mail, GP Dr Martin Scurr answered a question about leg ulcers (http://www.dailymail.co.uk/health/article-2235427/Ask-doctor-Whats-best-way-prevent-leg-ulcer.html).

In it, he quite correctly pointes out that leg ulcers can be cured by compression dressing and that they almost always come back again, needing continual compression.

What he didn't mention was the results of the ESCHAR study showing that surgery to the veins of the legs is just as effective in curing venous leg ulcers, but also prevents them coming back.

In our own research, we have cured 84% of leg ulcers - and 50% never need to wear compression ever again.

Mark Whiteley wins veins debate in New York

This morning, Mark Whiteley was involved in a debate against Steve Elias from the USA. The debate was about whether vein surgeons should treat perforator veins or not.

Mark Whiteley and Judy Holdstock from The Whiteley Clinic invented the method of ablating incompetent perforating veins in 2001 - their technique being called "TRLOP" - or TRansLuminal Occlusion of Perforators.

Steve Elias then subsequently renamed the technique "PAPS" - Perforator Ablation Procedure.

The debate lined up as:

  • Perforator Ablation IS Overused  - Steve Elias
  • Perforator Ablation IS NOT Overused - Mark Whiteley
The debate from Steve Elias was that he accepted that Incompetent Perforator Veins need to be ablated - nut only selectively.

Mark Whiteley argued that there is very good evidence that Incompetent Perforator Veins can be treated, reduce recurrence of ulcers and excellent circumstantial evidence that NOT treating them is a major cause of recurrent varicose veins.

The audience vote was decisive - 90% agreed with Mark Whiteley that Perforator Ablation is NOT overused.

This is excellent support from international vein experts who agree with one of the most important differences in the treatment for varicose veins and leg ulcers that The Whiteley Clinic and The Whiteley Protocol offers over that offered by most vein doctors and clinics.

Leg ulcer report in Daily Mail misses point

Today a report in The Daily Mail (link below) reported on new research showing that a "mesh" of skin cells can help to cure leg ulcers.

Although this, and anything else that helps leg ulcers, is of great value, the article missed the most important point about curing leg ulcers:

- If you find out the underlying cause and cure that, then the leg ulcers will heal by themselves in the majority of patients.

Most leg ulcers in the UK are caused by venous reflux ("hidden varicose veins"). If everyone who had a leg ulcer has a duplex ultrasound scan, and then those with venous problems were fixed, and the few with arterial problems were also fixed, then the majority of leg ulcers would heal without needing such new techniques.

This new technique is only needed for those ulcers that we can't heal  ....  The problem that should be shouted about in the national media isn't the need for a new product, but WHY AREN'T PEOPLE WITH LEG ULCERS BEING INVESTIGATED FOR VENOUS DISEASE AND OFFERED CURATIVE SURGERY?

"Superglue" for varicose veins looking good at 3 months

Last week we had 7 patients back for review and scanning having had the Venseal "Superglue" treatment of their varicose veins 3 months ago.

The full results will be published as part of a larger multicentre study.

However all of the patients were very happy with the treatment - and the results looked great on scan.

Although it is early days with this new technology, the results are looking very good and with our experience of what scans look like, we suspect that the long term results will also be just as good.

Therefore we are finding the Venaseal "Superglue" treatment of varicose veins to be a very promising addition to the new minimally invasive, local anaesthetic, endovenous treatments of varicose veins.

Yesterday, our patient bein filmed for Channel 4's embarrassing bodies had her pelvic veins embolised by Dr Tony Lopez of The Imaging Clinic.

She will be returning to The Whiteley Clinic next week for her severe recurrent varicose veins of the legs to be treated.

Fliming Embarrassing Bodies at The Whiteley Clinic

Today, and for the next couple of weeks, we are filming with Channel 4's Embarrassing Bodies show at The Whiteley Clinic in Guildford.

Filming today consisted of filming the patient explaining her problem, Duplex Ultrasound Examination and the discussion of the results with the patient.

All of the treatment options were explained to the patient on camera and she was consented to treatment.

She will be having her treatment at The Whiteley Clinic over the next couple of weeks

Recurrent varicose veins - pelvic venous reflux is ignored by most

Alice Whiteley is one of two Whiteley Clinic Summer Research Fellows who have had their research selected for presentation at The Medical Acorn Foundation (National Medical Student Research Conference) in Leicester on 13th October 2012.

Presenting her research into the cause of recurrent varicose veins operated on elsewhere 
Alice spent her summer analysing the results from patients seeking help from The Whiteley Clinic having had varicose vein surgery elsewhere and then getting recurrent varicose veins.

She identified the causes of their recurrent varicose veins, and found that pelvic venous reflux is one of the major causes, a problem that has previously been ignored.



 
Alice's research presents powerful evidence for why the pelvic veins need to be scanned and, if refluxing, treated, to reduce the risks of recurrent varicose veins in the future.

Her work has been submitted for an American meeting next year and will be submittied for UK vein meetings next year, before submitting for publication.

10 year results of Radiofrequency Ablation of Varicose Veins


Daniel Taylor is one of two of The Whiteley Clinic Summer Research Fellows from 2012 have had their research into varicose veins accepted for presentation at The Medical Acorn Foundation (National Medical Student Research Conference) meeting on 13th October 2012.

Daniel Taylor has spent this summer recalling pateints who were treated over 10 years ago at The Whiteley Clinic with the VNUS Closure Radiofrequency Ablation system and then analysing all of the 10 years scans of their legs.
 
He has been finding out what the results of this technique are and, if there were any problems, where they came from. The results appear to be the first 10 year results available from endovenous surgery.
 
Daniel has also submitted his research for an American Venous meeting, and will be submitting the same for a UK venous meeting prior to submitting the results for publication.

Laser closure of large varicose veins - with radial EVLA


Endovenous surgery (or "keyhole surgery") is taking over from open surgery for varicose veins, as this approach has a great many advantages. ONe of the biggest is the fact that endovenous surgery is performed under local anaesthetic and so the patients are walking home an hour after the procedure.
 
Although "normal" sized varicose veins are easy to treat by experienced vein experts with endovenous techniques, there has always been a concern about the maximum size that can be treated this way.

These images are ultrasound pictures taken yesterday of one of our patients treated 5 weeks ago at The Whiteley Clinic. The man had a vein that was 31mm in diameter (normal size in varicose veins at this point is between about 4-12mm).

The veins was successfully closed by using a side-firing laser and passing the laser backwards and forwards thorough the dilated segment four times.

By using novel techniques, we have not yet had a patient with varicose veins that we cannot treat with endovenous techniques under local anaesthetic.

2012 Pre-medicine taster course

Delegates get to use ultrasound, practice stitching, get taught how to take blood and other practical skills to see if they like Medicine and can see it as their future career.

The course ends with a mock medical school interview to help delegates prepare for applying to a medical college.

The course runs on Wednesday evenings during the Autumn term. There are a few places left for any interested students.

Removal of arm veins - a "Life changing" operation


A lady contacted The Whiteley Clinic earlier this year by e-mail, showing enlarged arm veins on both sides. She was desperate for removal of the veins as the veins stood out so much.

After much discussion, we agreed to perform Ultrasound guided removal of the veins, under local anaesthetic.

2 months later, the patient is delighted with the results:

She stated that the results were "life-changing" as she is now able to wear what she likes and is totally comfortable with her arms being on show.

Whiteley Clinic Prestige Stakes 2012 at Goodwood

This year's Whiteley Clinic Prestige Stakes, held at Goodwood on Saturday 25th of August, was a fabulous affair - despite the cloudy weather. Immaculately organised by Graham Smith of RT-Design, the day was a great success and was enjoyed by all:

The day started off with a flight into Goodwood with Capt. Paul Andrews of the Phoenix Helicopter Academy, based at Goodwood but covering the whole of southern England.

Having flown over 45 hours now, I flew most of the way - but due to the gusts, Paul performed the landing at Goodwood racecourse.

Before the race, 6 of us were invited to go to the parade / winners circle to choose the best turned out horse of the race:

The best turned out horse was Sky Lantern and the prize was given for the stable boy before the race began.

We then went to watch the race from the top of the Grandstand giving fabulous views of the race:
